Pre-surgery Process & Information

The first step in the pre-surgery process would be to register for a free informational seminar. It is important to get information about the different weight loss surgery options.  In conjuction with your consult with Dr. Srikanth, the seminar information will enable you to choose the right surgery for you.

Complete the Information for Insurance Verification Form.  Please send the completed Insurance Verification form as well as a copy of the front and back of your insurance card to our office in Federal Way or to our secured fax (253) 815-7708.   Please do not send the information via email (see our Privacy Policy for more information).  Our office will verify insurance coverage and get back to you to schedule your consult with the surgeon.

	Complete the Comprehensive Patient History Form and send it to our office.
	Use the  Release of Information Authorization Form and send it directly to other doctor’s offices and facilities to request records for weight history, study reports, etc.
	If you have had weight loss surgery and are considering revision surgery please send us a copy of the previous operative report.
