The Process to Get Started

Thank you for visiting the web site for the Center For Weight Loss Surgery (CWLS). It is our desire that you become well informed regarding Bariatric Surgery and the new life that is available to you because of these procedures.

You will see that surgery through CWLS is basically a 5 Step Process which is outlined in the Steps to Surgery that can be downloaded from this website. The journey with our practice begins when you contact the Northwest Center for Weight Management at St. Francis Hospital in Federal Way, and they register you to attend (at no charge) their Community Seminar (888-825-3227). Just make sure when speaking with them that you request a session when Dr. M. S. Srikanth will be speaking and answering questions.

If you have already attended a Community Seminar, or have been waiting for Dr. Srikanth to open the doors of his own practice, the waiting is over and you can download paperwork necessary to begin the process with the Center For Weight Loss Surgery. There are five forms ready that you may need:

You will need Adobe Reader to access these forms. 

 Information for Insurance Verification - Complete this form so we can contact your insurance company.
 Understanding Insurance - Information to help you work with your insurance company.
 Steps to Surgery - Guide to the complete process in having surgery with CWLS.
 Comprehensive Patient History - This must be complete; and it gives instruction for the Release of Information
 Release of Information Authorization - Make as many copies as you need to obtain necessary medical information.

 Remember: Step One for everyone is to attend the Community Seminar with Dr. Srikanth at the Northwest Center for Weight Management. You can download and complete the necessary paperwork at any time, but please do not send it to the office until after you have met Dr. Srikanth.

Insurance Pay: If you want to use insurance coverage for surgery, completely fill out the Information for Insurance Verification and send it to our office. Send it with a copy of the front and back of your insurance card. As soon as those are received, we will verify with your insurance company and call you. OR, you may bring your completed Information for Insurance Verification to the Community Seminar and give the form and a copy of your insurance card, front and back, to Dr. Srikanth that evening.

We will call you within that week to discuss your insurance options. Once you have submitted your insurance information to our office, you may want to work on the Comprehensive Patient History so that it is ready before your initial office visit.

Cash Pay: If you are going to pay cash for your surgery, then you will not need the two insurance pages. Download the Steps to Surgery, the Comprehensive Patient History and the Release of Information Authorization.  Call the Northwest Center for Weight Management to register for the required session.

For those of you who have already attended a Community Seminar and are ready to start working toward surgery, just download the Steps to Surgery, the Comprehensive Patient History and the Release of Information Authorization forms. Call our office if you have any questions and/or to schedule your initial visit.
